White Sticker

I will being getting my plates tomorrow and I want to send in the form to get my white sticker ASAP. I had a question on completing Section A. The dealer pre-filled out some of the form, under True Full Name it list Toyota Lease TRST, do I just put my address on the form or to I list myself as co-owner and my address? I have never leased a car before so I am not sure on how to complete the form.

I would suggest downloading a blank form from the DMV and putting your name and address on there. That's what I did for mine.

i filled out a form myself.
I don't recall putting toyota financial services/ lease trust.
